장애인 맞춤형교정용신발 정부의료급여 지원 받으세요!
■ 장애인 맞춤형교정용신발이란
뇌병변장애 또는 지체장애인 중에서 발의변형 또는 다리의 길이차이로 보행 장애가 있는 분들의
보행 교정을 위하여 본인의 발에 맞도록 제작하여 신는 신발을 맞춤형교정용신발이라 합니다.
■ 제작업체 소개
당사는 특허 받은 중창을 각각의 장애유형에 따라 적용하여 편안함, 자세교정, 디자인을
모두 만족시켜 기존의 장애인 맞춤형교정용신발의 불편한 점들을 획기적으로 개선한 수제 맞춤형교
정용신발을 출시하였습니다.
■ 구입 지원 절차
1. 본인의 의료급여종류(기초생활수급, 차상위, 건강보험가입)와 장애종류(뇌병변, 지체장애, 기타
중복 장애)를 먼저 확인합니다.
2. 정형외과, 신경외과, 재활의학과가 있는 병원으로 수급자용 처방전 또는 건보자용 처방전을 잘
구분하여 지참하시고 해당 병원에 방문하여 맞춤형교정용신발 처방전을 발급받습니다.
3. 발급받은 해당 처방전을 기초수급, 차상위 장애인은 본인의 주소지 관할 읍, 면, 동사무소에
제출하시고 건강보험가입 장애인은 바로 맞춤형교정용신발 제작의뢰를 하시면 됩니다.
■ 구입 지원 금액
1. 기초수급 장애인 : 250,000원 전액 지원하여 드립니다.
2. 건보가입 장애인 : 225,000원 지원 25,000원 자부담하셔야합니다.
■ 처방전 기재 내용
1. 상기인은 우측 또는 좌측 족부변형으로 상기 보장구가 필요하다고 사료됨
2. 상기인은 우측 또는 좌측 하지의 단축으로 상기 보장구가 필요하다고 사료됨
■ 구입 가능 장애유형 및 등급
뇌병변 또는 지체장애로 보행 장애를 격고 계신 장애인이시면 누구나 가능합니다.
※ 처방전 발급을 위하여 해당 병원에 가시기전에 본인이 하반신 장애로 복지카드에 기재가 되어
있는지를 꼭 확인하시고 가세요!
